    In compliance with the Personal Data Protection Act ("PDPA"), Cryoviva Singapore Pte Ltd seeks your consent to collect your NRIC in order to maintain the records of the umbilical Cord Blood/ Cord Tissue to be transferred and stored with us in our MOH licensed laboratory.
    The nature of business is such that it would require this information for identification of the unit in future during retrieval and such medical events and is deemed necessary for it.
    The company is in compliance with the PDPA and this information would be kept highly confidential and would not be used for any purpose other than mentioned in this form.
